Handle underscores which surround the full name, not just a part of
it. In this case, then the whole thing should be considered as the name,
and the last name should be left nil.

Some items are going to be really empty: just the title and the author;
no year, no notes, etc. In these cases the exporter still tried to add
this missing information.
